HomeMy WebLinkAbout04242017 City Council Laydowns ii).°411.11141. °COMMUNITY <HEALTH CENTER Report to City Council April 24, 2017 Patient Activity for January— Dec 2016: • 1,935 Unduplicated Patients (9.2% increase over 2015 — level from Sept - Dec) • 5,004 Patient Visits with Providers (27.8% increase over 2015) Financial Highlights for FY 2016 (unaudited): • SCHC is completing its second independent financial audit with BDO for FY 2016. The audit report should be received in the next month or two. An unaudited positive bottom line of$141,558 was realized for 2016, and we finished the year again with a good balance sheet position. • Through three months of FY 2017, SCHC has a negative bottom line of$142,141. This is primarily due to three things: the usual January draw down of the federal 330 grant was accelerated into December 2016 as total allowable and budgeted expenses for the grant year had been incurred by 12/31/16 (with no draw done in January); the first quarter of the year is historically a low volume period compared to the second and third busy season quarters, and; we historically incur a number of material, one-time operating expenses that are paid in the first quarter of the year. • The Health Center transitioned to a new billing service in September 2016 in preparation to its conversion to the Epic electronic health record system on October 31, 2016. The new service provider is proficient on Epic and the legacy system. • One additional one-time, supplemental grant for $18,886 was received from HRSA since my last report to the Council. This was for achieving improvements in our quality assurance metrics that are part of our UDS reports. These funds were also used to help pay for the up-front costs associated with the conversion to Epic. • The conversion to Epic is through an EHR sharing program provided by Providence Health and Services. We estimate that SCHC will realize savings in annual operating costs of over $50,000 per year as a result. • The Health Center implemented a 340B federal drug discount program on October 1, 2016 working with Safeway/Albertsons as our contracted pharmacy. In our first three (and slower volume) months of October through December, SCHC realized net revenues of $7,000 per month on average. APRIL 2017 SCHC REPORT TO CITY COUNCIL Current Provider and Support Staffing: • SCHC employed providers remain stable with two full-time family medicine physicians (Dr. Van Camp and Dr. Allen) and two full-time Physician Assistants (Jeanette Nienaber and Amy Knissel). • No new positions have been added since the last report to the City Council. Employment remains steady at 20 FTE's in total. • The main position being turned over at the end of this week is the Executive Director due to my retirement. Craig Ambrosiani has been selected by the Board as my successor. This is his first day on the job. • Dr. Jodi Elliott, pediatrician, has joined our provider staff on a part-time contracted basis. She has held two outreach clinics so far in 2017 which have gone well. Major Projects and Initiatives: • The major project the past several months has been the conversion of our electronic health record system from NextGen, our previous vendor, to Epic through a very beneficial "sharing"program offered through Providence Health and Services. We have received excellent support from the Providence Community Connect Team since the start of this project and the conversion has gone relatively very smoothly. We now have bi-directional interoperability with the hospital lab and radiology departments for orders and results. We can also follow and track the care our patients receive in real time if they are in any Providence or Community Connect facility or being seen by any associated providers. Our patients can also see their clinical information through the secure MyChart patient portal. • As an FQHC, we are eligible to participate for medical malpractice coverage in the Federal Torts Claim Act (FTCA) program. This major application was submitted at the end of March, and we await the news that we have met the requirements and have been accepted into the program. • In conjunction with a $35,000 supplemental grant we received from HRSA last year, we are now working diligently on our application to receive national certification as a Patient Centered Medical Home. Much advance work is now being done with an anticipated submission date of July 27m • The Board has been active in its negotiations to purchase the .9 acre parcel immediately north of the hospital campus. Closing on this property is expected in the next few weeks. This will be the site of a new Health Center facility. Project planning and researching funding strategies will take place over the next couple of years. Stay tuned as this exciting project for the Health Center, our patients and the community unfolds. Other Highlights and Points of Interest: • Every three years HRSA conducts a three-day Operational Site Visit with a team of reviewers. The next OSV is scheduled for August of this year. This will be preceded by a mock survey conducted by a team from our state association in early June. This resolution shall take effect immediately upon adoption. .4(24(,1 Report to Council 24 APRIL 2017 I,y(to Seward Chamber of Commerce, CVB Cindy Clock Chamber staff volunteered at the Seward Job Fair and at the Financial Reality Fair at SHS this month. Kris & Margee staffed the Seward booth at the last sports & rec show of the season in Fairbanks. We've been preparing for an audit next month by Lambe Tuter & Wagner. We do face some challenges with only three full time staff, but we have the Chamber Board Treasurer, Melissa Schutter reviewing all bank & credit card statements & reconciliations, payroll taxes, and profit/loss statements each quarter. Von Terry makes up the other half of the Finance committee — Von also reviews the profit/loss and budget --- or Profit Plan, as I like to say :) The Seward Chamber has hired Shared HR, a firm based in Bellevue, WA to address our human relations needs and questions. Lynette Hatscher is available to us for 3 hours a month. As she says, whether an organization consists of 1000 or just 3 — the HR issues remain the same. She has already proven to be a valuable resource addressing annual evaluations, job descriptions and we'll soon hear about "Growth Coaching" and success plans. Because we don't make Kris do the bookkeeping, she is free to concentrate on Member Services. I have recently been calling her a `Member Magnet'. We now boast 384 active members — up from the range of 324 — 340 over the past few years. Denny Murphy is our part time bookkeeper. Last fall I separated the Public Relations part of the PR & Events Desk allowing GeNeil to concentrate full-time on events organization. She is a much happier employee, and we know that Yvette Galbraith with Alaska Marketing Consultants is only too capable of promoting all we do. Charity Cinereski transitions from student intern next month to Visitor Center rep, and Jen Leahy will be working at the Derby Booth during the June Halibut Tournament. Jen is the Communications & Engagement Manager in Seward for the Alaska Marine Conservation Council. The Treaty with Russia was negotiated and signed by Secretary of State William Seward and Russian Minister to the United States Edouard de Stoeckl. In honor of this 150th anniversary — there'll be a "Name the July 4th Parade" contest at Seward Elementary. Could possibly be extended to the other schools. WORK BY OTHERS (NOT INCLUDED IN CONTRACT PRICE) Main Line Power Feeders The current main line piping and wiring from the disconnect to the new controller location M�ay need to be relocated to accommodate the new controllers. A ground wire must be added if it does not already exist. ERU requires auxiliary contact in disconnect. Cab Lighting A single 110 circuit must be run to each controller. This circuit must be run through a disconnect or switch that is lockable in the on or off position. The location of this switch must be coordinated with the elevator mechanic on site. GFI Outlets . AS All Outlets in the machine room and/or the pits must be retrofitted with GFI outlets per code. Light Guards All lighting in the machine room and/or the pit must be retrofitted with cede compliant guards made of either grounded metal, plastic or comparable. Phone Code requires a single phone line per elevator to be run in conduit to each controller. This line must be a normal dial tone outside line. Unless otherwise agreed upon by the elevator contractor. Disconnects The main power elevator disconnects must be code compliant, fused or breakers, and must be lockable in the off positions. The disconnects must be located within clear site of each elevator motor. Smoke detectors must he installed if not 0 currently operating, at every lobby, the machine room, and the hoistway(if sprinklers are present). Smoke detectors in the (n hoistway must be mounted on an access panel that is serviceable from the outside of the hoistway. This panel must be no greater than 2' x 2'. 0 Shunt trip Disconnect If sprinklers are altered or installed in the machine room and or the D hoistway as part of the modernization, a heat detector must be rn installed within 18"of every sprinkler head in the machine room and the hoistway overhead. These heat detectors must activate a shunt trip disconnect located outside the elevator machine room, to (V remove all of the three phase elevator power prior to the power entering the machine room. It is Purchaser's sole responsibility to coordinate the performance of these items with ThyssenKrupp Elevator to ensure a successful completion of this project.The following is a list of those items that are not included in this Proposal: I. Equipment Storage:the provision of a dry and secure area at the project site for storage of the elevator equipment at the time of delivery and the provision of adequate ingress and egress to this area. Any relocation of the equipment as directed by the Purchaser after its initial delivery will be at Purchaser's sole expense 2. Electrical: a. suitable connections from the power main to each controller and signal equipment feeders as required,including necessary circuit breakers and fused mainline disconnect switches per N.E.C. Suitable power supply capable of operating the new elevator equipment under all conditions; b. the wiring to the controller for car lighting per N.E.C.Articles 620-22 and 620-51; c. a means to automatically disconnect the main line and the emergency power supply to the elevator prior to the application of water in the elevator machine room that shall not be self-resetting; d. wiring and conduit from life safety panel or any other monitor station to the elevator machine room or a suitable connection point in hoistway; e. a bonded ground wire,properly sized,from the elevator controller(s)to the primary building ground;and all remote wiring to the outside alarm bell as requested by all applicable code provisions f. a dry set of contacts which close 20 seconds prior to the transfer from normal power to emergency power or from emergency power to normal power whether in test mode or normal operating conditions in the event that an emergency power supply will be provided for the elevator; g. automatic time delay transfer switch and auxiliary contacts with wiring to the designated elevator controller and h. electrical cross connections between elevator machine rooms for emergency power purposes i. the following emergency power provisions are not included: interface in controller,pre-testing and testing,emergency power keyswitches; 3. Machine Room:a legal machine room,adequate for the elevator equipment,including floors,trap doors,gratings,foundations, lighting and a machine room temperature maintained between 50 and 90 degrees Fahrenheit,with a relative humidity less than 95%non-condensing; 4. Heat and Smoke Sensing Devices:heat and smoke sensing devices at elevator lobbies on each floor,machine room,and hoistways with normally open dry contacts terminating at a properly marked terminal in the elevator controller; 5.
