3 PROVISIONS:
4 1. Persons who may vote absentee: Any qualified voter may vote absentee at
5
any municipal election,
6
a. If he will be unable to be present at the polls because of physical
7
disability or
8
b. If he will be unable to be present at the polls because of the physical
9 inaccessibility of the polling place causing undue hardship or hazard
10 to the voter.
11 2. Provisions for the general administrative supervisicm:
12
a. The City Clerk shall provide general administrative supervision over
13
the conduct of absentee voting and may issue any rules necessary for
14
the administration of absentee voting to insure efficiency and voter
15
participation. The City Clerk shall issue instructions to absentee
16 voters regarding the procedure for absentee voting. One set of instructions
17 shall accompany each absentee ballot.
18 3. Preparation of ballots, envelopes and other material:
19
a. The City Clerk _m provide the paper ballots prepared for use at the
polls as the absentee ballots. The City Clerk shall provide a small
20

blank envelope in which the voter shall initially place the marked
ballot and shall provide a larger envelope with the prescribed voter's
22
23
certificate on the back in which the smaller envelope witJy ballot en-
24
closed shall be placed. The City CleN shall prescribe the form. of and
25
prepare the voter's certificate, envelopes and other material used in
26
absentee voting. The voter's certificate shall include an oath, for use
27 when required, that the voter is a Itllalified voter in all respects, a
28 blank for the voter's signature, a certification that the affiant prop-
29 erly executed the marking of the ballot and identified himself, and.
30 blanks for the attesting witnesses.
31 4. Distribution of ballots, envelopes and other materials:

a. The City Clerk shall distribute the absentee ballots, envelopes and

1 other absentee voting E~terial to the election board at the beginning of the
2 day of the election.
3 5. Requirement for f!lll public noti_(~e:
4 a. The City Clerk shall give f\1ll public notice of the dates ani manner of voting
5 absentee and ma~' select an~r l'leans of communication permitted to be used in
6 giving notice of the date and time of the ~eaal election.
7 6. Any ,-!ualified voter may apply hy personal representative for an absentee ballot
8 to the election board. This application shall be signed by the applicant and
9 delivered by personal representative to the election board on the date of the
10 election. The application shall include the name of the applicant, his address,
11 the reason for being unable to reach the poDing place. This shall be accompanied
12 by a letter from a licensed physician or a statement signed by two ~ualified voters
13 stating that the applicant will be unable to go to the polling place because of
14 physical disability or that such a trip would be an undue hardship or hazard to
15 the voter.
16 7. Procedure on application by personal representative:
17 a. Upon receipt of an~r written application by personal representative the election
18 board shall provide the ballot and other absentee voting material if the
19 written application is signed by the applicant and is accompanied by a letter
20 from a licensed physician or a statement signed by two qualified voters stating
21 that the applicant will be unable to go to the polling place because of phy-
22 sical disability or undue hardship or hazard to the voter due to the physical
23 inacceel!libili ty of the polling place to the voter.
24 8. Responsibility of the Election Board:
25 a. Prior to issuing an absentee ballot for an applicant by personal represen-
26 tative, it shall be the duty of the election board to determine that the
27 applicant is a yualified elector in accordance with the requirements for
28 eacb type of ballot to be issued.
29 9. Casting vote by personal representative:
30 a. Upon receipt of an absentee ballot through a personal representative, the
31 voter, in the presence of two attesting witnesses, both of whom are quali-
32 fied voters, or before any election judge, notary public, commissioned

officer of the armed forces including the National Guard, any district magistrate
or deputy magistrate, United states postmaster, United states assistant post-
master, or other person ~ualified to administer oaths, may proceed to mark the
ballot in secret, to place the ballot in the small blank envelope, to place the
small envelope in the larger envelope, and to sign the voter's certificate on
the back of the lareer envelope in the presence of the above-listed official
or described persons who shall sign as attesting witnesses. The voter may
then return the ballot properly enclosed in the envelopes I b:lr personal repre-
sentative to the election board who provided the ballot not later than the day
of the election.
Payment of fee:
a. No person may receive a fee from the voter for attesting to any voterJs
certificate required in voting absentee.
14 11. Preparation of voting list:
15 a. The election board shall prepare for public inspection a current list of the
16 names and addresses of persons who have voted absentee.
17 l~. Disposition of ballots:
18 a. Absentee ballots shall be plaCed in a separate ballot box and counted and
19 shown separate from the other tabulations.
20 13. Effective date:
22
21 a. This procedure shall be placed into effect as soon as possible.
