Sponsored by: Oates <br /> CITY OF SEWARD, ALASKA <br /> RESOLUTION 2009 -023 <br /> A RESOLUTION OF THE SEWARD CITY COUNCIL OF THE CITY OF <br /> SEWARD, ALASKA, PROCLAIMING THE CITY OF SEWARD AS THE <br /> WELLNESS CAPITAL OF ALASKA AND AUTHORIZING THE CITY <br /> MANAGER TO SEEK STATE AND FEDERAL RECOGNITION AS THE <br /> WELLNESS CAPITAL OF ALASKA AND APPROPRIATE FUNDS FOR <br /> A DESIGNATED WELLNESS FUND UNDER THE SEWARD <br /> COMMUNITY FOUNDATION AND SEEK PARTNERS FROM <br /> NATIONAL SERVICE PROGRAMS (AMERICORP AND VISTA) TO <br /> DEVELOP, ENHANCE AND IMPLEMENT WELLNESS INITIATIVES <br /> WITHIN THE CITY OF SEWARD <br /> WHEREAS, the City's hospital and Long Term Care Facility have a vested interest to <br /> provide quality health care services; and <br /> WHEREAS, the City of Seward entered into an operating agreement with Providence <br /> Health and Services, Alaska Region to manage the City's hospital called the Providence Seward <br /> Medical and Care Center; and <br /> WHEREAS, Providence Seward Medical and Care Center completed a community <br /> ,,. needs assessment on April 8, 2008; and <br /> WHEREAS, as a result of this community needs assessment, a community wellness for <br /> all group was formed including a cross section of Seward residents and health care providers; <br /> and <br /> WHEREAS, the wellness for all group identified five wellness categories to further <br /> identify and target wellness for all; and <br /> WHEREAS, each category formed a group and assigned a lead position that would <br /> further define and develop programs to improve wellness for all; and <br /> WHEREAS, each group has developed wellness strategies, goals and programs to create <br /> a state of the art wellness model; and <br /> WHEREAS, a small grant has been completed and submitted to further develop and <br /> expand these programs; and <br /> WHEREAS, the intent of the grant is to collect community wellness data including a <br /> know your numbers program and to develop a patient centered medical home business plan and <br /> develop a wellness neighborhood model as a one year, pilot program before full implementation; <br /> and <br />
