Interim Executive Director's Monthly Report February 20, 2014 As we grow closer each day to the opening I realize that while we have accomplished a lot, there is still more to be done. Here is break down of what has happened since my last Board report. Delaying the opening to March 10 has provided us with the extra time needed to paint, work on hiring staff, and the ordering and receiving of equipment and supplies. Staffing: We have hired two front desk staff, a outreach worker, an administrative assistant and nurse( who will be on board end of March). As you can see we are still lacking medical support staff. I did have one who I offered the position to but she gads since declined. I have done reference checks and the City is handling the drug screen and background checks. They have been informed that while they have been offered a position it isn't final until both the background check and drug screen come back with a positive report. Once they clear the background and drug screen I will make an office announcement to the public, and will pass names on to the Board. We have contracted with a PA locum to start seeing patients, starting March 10. She will only be here for 10 days but may be back in April. I am still working on other locums and permanent providers. I am also working on additional medical support staff. Staff that applied but did not receive and interview or did not make the cut after the interview were contacted, and encouraged to apply as other positions become available. In talking to my colleagues across the State, there are many clinics trying to hire providers and having the same difficulties we are having. I say this so you know that we are not unique in this nor is it something that we are doing wrong, I think the pool is small right now. I have asked all staff to report for work on March 3. We have a lot to do that week in preparing charts, setting up a schedule book, making sure all exam room are supplied and that all the equipment is working. I know there was a question about staff training and what training will be done. I am setting up an online training program for staff. Through this program staff can go online take the appropriate training and receive a certificate of completion, which is put in their charts. Basic training, HIPAA, Blood Borne Pathogens, OSHA, and Sexual Harassment/Work Place Violence. During this training period we will be reviewing job descriptions and the personal policies. There certainly are other trainings that we can set up for staff like costumer services, dealing with disgruntled customers, etc. and we will be will down the road. Space: The painters are in and working diligently behind closed/plastic sheets. They should be done at the end of the week. I was able to sneak in and took a peak at one of the exam rooms, and I think that everyone will be happy with color change, giving everything a fresh new look. PSMCC has done a good job in preparing the waiting room area and reception area by shampooing the rugs and washing the desks, etc. Mission:To Identify and serve the healthcare needs of our community by providing quality, affordable services and promoting wellness, prevention and partnerships Seward Community Health Center, Inc. We are still negotiating the cost that PMSCC will charge us for utilities, maintenance and cleaning. We are also negotiating a contract with them; leasing back two exam rooms that they believe they need for overflow in the ER. TelAlaska should have the phones in next week, and CCI should have the internet service in as well. *****One thing that we do need, that we or rather I have overlooked is a counter/long table in the room that will become the lab and some extra shelves. There might also be some small items like office chairs, but we will know that once we set up rooms. Equipment and Supplies: New exam room equipment should be in Anchorage on March 1 and down here in place on March 2. Henry Schein will remove any of the old equipment and exam room furnishings that PSMCC does not want. We are replacing the exam tables, chairs and exam table side lamps. Michelle has been working on a list of needed supplies and instruments, which will be ordered from either Henry Schein or Amerinet. We have agreed to pay PSMCC for unopened supplies in the rooms. This way we are assured that the rooms will adequately be stocked with supplies for March 10, should our supply order be delayed. Michelle has been working with Laura-Jean in coordinating the stocking of the rooms. On the administrative side, I am working on ordering the administrative supplies, which includes hard medical charts. We have also ordered computers and printers for both the admin side and medical staff. PR • I have the rack cards, which need to be distributed through out the community. • I have sent the delayed opening announcement to the local media • I attended the PSMCC Health Council meeting on Wednesday and based on some questions I think we need to do some hard core PR on what the CHC, to dispel an y rumors about how we are and what we will and will not do. • Would recommend setting up a Facebook Page • We also need to get the website up and running or develop a CHC Blog • I would recommend taking out an ad in the local papers (hard and online) at least for the first few months • Would recommend holding an informational evening, maybe March 3 for folks to answer questions and dispel misinformation floating around Community Connections I have been trying to get around the community and talk with folks and attend community functions to get the CHC word out to the public. • I attended the Rotary Meeting in February, handed out rack cards and field some questions Mission:To Identify and serve the healthcare needs of our community by providing quality, affordable services and promoting wellness, prevention and partnerships Seward Community Health Center, Inc. • I attended the PSMCC Health Council meeting in February, again passed out information and fielded questions • Attended the Rotary Wine Tasting Fundraiser • Met with Bob Shafer of the Seward Journal Finances I have been working with Kris and the Finance Committee as we review the ongoing expenses and modify the working budget to reflect them. According to my current calculations, this is the working 12 month budget (Feb 2014 through Jan 2015) hiring at the top of the salary line, we would be $7,800 over budget, this includes salaries, signing bonus and loan repayment (one year). Accordingly, have looked at income and if we can generate what we said in patient revenue ($586,000) and hold the expenses, we should be Ok in the first grant year. This is different than looking the fiscal year (Jan to Dec). Attached are two spreadsheets on is looking at the EMR/PM program and the other is contracts that are in place. Respectively submitted Sharon Montagnino Mission:To Identify and serve the healthcare needs of our community by providing quality, affordable services and promoting wellness, prevention and partnerships Seward Community Health Center, Inc. EMR/PM I have reviewed both programs and spoken with number of in State and out of State users and here is my assessment spy TP'tC v_ s More of an out patient/clinic built program, used by EPIC is in patient/hospital built program, however 4-5 CHCs in AK and in other States through OCHIN they are able to offer it to CHC Users report that the reporting is very robust and While it is a robust reporting system, getting the they are able to extract UDS data easily. OCHIN has built the which is common with most EMR/PM UDS data reporting for the CHCs and that appears to work well NexGen willing to roll out the PM and the in 2-3 EPIC/OCHIN will not roll them out separately. They months roll out the EMR. This what I am used to, maintain that it is best to o it all at once. Based on it is easier for staff. Front office learns the PM my experience I would disagree and from those (registration, scheduling, billing, etc, and then the who use it , they point their frustration to the way providers come on board with the EMR. The EMR the program is rolled out. and PM work together NexGen can have us up and rolling with the PM by OCHIN says November or December possibly into mid to end of April, at least the last time I spoke 2015 before they could get us up and running. with them that was the timeline. They also have There is major upgrade to the EPIC program an upgrade coming in April but it is on the EMR coming in April/May, so they do not want to bring side and will not have a major effect on the PM any new programs on until that is completed side. Billing- NexGen has a billing program. Some in Talked to those using OCHIN for billing and they AK had problems with the billing in the beginning, are not pleased, in fact there have been problems but that has been corrected. Now the CHC does so they are looking to pull away. the primary posting and send to them for the secondary-EOB Support-This is a major concern and need Support-This is major concern and need Again off the beginning, those with NexGen While we (Suzanne, Patty and myself) were able reported issues with lack of timely support. Since to speak with OCHIN folks early on about the then they have assigned one support person to all program and our needs, I had great difficulty AK CHCs. They are onsite for training during the making contact later on. I called OCHIN and instal and now have weekly online meetings with emailed them wanting o see a demo and asking the support group. One note here-from the person about how the program could be rolled out, but did trying to gather information, when I called NexGen, not hear back for days. Finally, though the efforts they returned calls promptly, available to show me of John and Joe, I was able to see a demo. Based and Michelle a demo on the program, maintained on the lack of response when inquiring about email contact to answer questions. purchasing the program makes me wonder how the support will be once we are using the program. I am hearing from those in State and out of State that OCH IN's support is lacking, they are overwhelmed with the amount of new starts and do not have the staff to handle the support needs. I would still recommend and support using the NexGen program, based on support, billing and a timely instal. With the UDS reporting that CHCs have to do, waiting a full year and trying to capture data by hand presents many problems. All programs have some good areas and some not so good areas.
